"Your account has been disabled" error when submitting a record for approval

Description
When I submit a record for approval or approving/rejecting an approval.  I am faced with the below error:

Your account has been disabled
Your company administrator has disabled access to the system for you. Please contact your administrator for more information.
Resolution

ROOT CAUSE:

This error is produced when a User selected as an Approver or their Delegated Approver is inactive. 

SOLUTION:

To remedy this, replace the inactive user in the Approval Process / Delegated Approver Field on the Approver's User record with an Active User.
