Native Live Agent - Pre-Chat Form - Connection Lost: Please check your local connection

Description I have created a Pre-Chat Form Visualforce Page, using the Pre-Chat Form Code Sample here: http://www.salesforce.com/us/developer/docs/live_agent_dev/Content/live_agent_pre_chat_forms_code_sample.htm

When the user submits my form they receive the following error:

Connection Lost: Please check your local connection

Resolution It is likely that you have not updated the Chat Button Id's with your own Id's

Code Sample:
      Department: <select name="liveagent.prechat.buttons"> 
          <!-- Values are LiveChatButton IDs. -->
          <option value="573a00000000001">Customer Service</option> 
          <option value="573a00000000002">Technical Support</option> 
          <option value="573a00000000001,573a00000000002">Customer Service if online, 
          otherwise Technical Support</option> 
      </select><br /> 

You will need to edit your Visualforce Page and update the Chat Button Id's with your own values.

These can be found from Setup | Customize | Live Agent | Chat Buttons.
