INSUFFICIENT_ACCESS_ON_CROSS_REFERENCE_ENTITY on record creation

Description When attempting to create a case or lead via API or Apex, you may see an error like,  'INSUFFICIENT_ACCESS_ON_CROSS_REFERENCE_ENTITY'.  This generally happens if the email template associated with Case/Lead Assignment rules is inaccessible to the user.
Resolution
To resolve, grant required access to the user for the folder in which the template is stored.  This can be done from [Your name] > Setup > Administration Setup > Communication Templates > Email Templates.  Select the folder in which the template resides, from the picklist, then click Edit.  Choose the desired level of access to the folder and save.
