INSUFFICIENT_ACCESS_ON_CROSS_REFERENCE_ENTITY on record creation

|Description||When attempting to create a case or lead via API or Apex, you may see an error like, 'INSUFFICIENT_ACCESS_ON_CROSS_REFERENCE_ENTITY'. This generally happens if the email template associated with Case/Lead Assignment rules is inaccessible to the user.|
To resolve, grant required access to the user for the folder in which the template is stored. This can be done from [Your name] > Setup > Administration Setup > Communication Templates > Email Templates. Select the folder in which the template resides, from the picklist, then click Edit. Choose the desired level of access to the folder and save.
