Live Agent visitor window not rendering properly

LiveAgent visitor pop up window continuously shows that it's waiting for an available Agent after a visitor initiates a chat. This issue is constantly reproducible and occurs when the Org has Pre-chat forms on the Live Agent button configuration.
To further verify the issue, please open the custom chat window Visualforce page used for LiveAgent to see if it is linking to Salesforce's cascading style sheet (CSS). You should see a line similar to this:
