Do you have the Account Engagement permission set assigned? Check by reviewing Give Users Access to Account Engagement Data in Salesforce.  

Is your CRM username linked and verified in Account Engagement? Review our documentation on user configuration to find out. 

You've cleared your browser's cookies/cache and/or tried another browser, closed the browser and re-opened, restarted your computer, etc. What else could be causing this?

It is possible that your Salesforce Org is blocking Account Engagement SSO from functioning. Advise your Salesforce Admin to do the following in Salesforce: 
1. Navigate to Setup | Manage Apps| Connected Apps OAuth Usage
2. Look for 'Account Engagement SSO' in the list
3. Under Actions (to the right), you should have the option to Unblock/Block and Install/Uninstall. Either unblock or install if need be.

This issue can also be caused by having restricted IP logins in Salesforce. If this feature is enabled, work with your Salesforce admin to ensure the Account Engagement IPs are added to the allowlist.

NOTE: Firefox 72 released a new feature called "Enhanced Tracking Protection.
When using Account Engagement in Salesforce Classic and clicking the 'Add to Account Engagement list', Account Engagement will be blocked and the unspecified error will be returned. Consider disabling enhanced tracking entirely or add Account Engagement as a safe site.
