In Salesforce, the enhanced Notes feature (introduced in Summer '15) creates a Note record as soon as the 'New Note' button is clicked — before any content is entered. This means that if a user opens a new note and clicks Cancel (in Salesforce Classic) or the X button (in Lightning Experience) without entering content, a blank Note record is still created and saved. This behavior is working as designed.
If new Notes is enabled, you will see the “New Note” button in the “Notes” related list. The following is expected behavior:
If a blank or unwanted Note record was created, navigate to the Notes related list on the record, locate the note, and select Delete from the row action menu to remove it.
The cancel button in Salesforce Classic and the 'X' button in Lightning Experience will discard any changes made from clicking the “New Note” button. The new record though is created when you click on “New Note” button. So if the New Note Button is clicked and nothing is entered and you click cancel, a blank note will be created.
If you need to remove the newly created record, please click the delete option from the “Notes” related list.
