There are a number of reasons why the 'Send Email' button does not appear in the 'Activity' related list in Classic or as Chatter action in Lightning even after it has been added to the page layout.
First, check if the 'Access to Send Email' level in the organization is set to 'No Access' or 'System Email Only'. If these system permissions are enabled, administrators are to review the profile settings of the affected users to ensure that the 'Send Email' permission has been enabled.

After changing this setting, the 'Send Email' button becomes available from the 'Activity History' related list in Classic or as a Chatter action in Lightning.
