Salesforce will not sync and provides an error after entering the Login credentials.

Unable to connect to Salesforce. 

Unable to access your outlook data. Review the appropriate log for details.

SQLite error
table SyncEntityConfiguration has no column named syncFolderName.
The steps below can also help in a situation where where the error "database file is locked" can be seen in the Salesforce for Outlook UI or in the Salesforce for Outlook logs.   
Résolution
  1. Close both Outlook and the Salesforce for Outlook plug-in. To close the plug-in right click on the icon in your system tray and select 'Exit'.
  2. Click start and type %AppData%\Salesforce.com\Salesforce for Outlook in the Search Bar.
  3. Right click on DB folder and rename it example: DB old.
  4. Open Outlook and wait until it loads up. 
  5. Open the Salesforce for Outlook plug-in.
  6. You'll receive a prompt to sign in with your Salesforce Username and password, if not you would notice the Salesforce for Outlook icon in the system tray is grayed out. Simply right click on Salesforce for Outlook system tray icon, select Settings and you will receive a prompt to sign in. Choose the Change URL drop down on top if you are using a custom domain, enter the URL and then click on Okay
  7. Enter the Login credentials and click on Login to Salesforce.
  8. Click Allow.
  9. Click Next and check the sync folders. You can use the Change Folder button to choose or create a new sync folder
  10. Select the type of sync method i.e. Sync All Outlook Items or Sync Only the Outlook Items I Select and click Next
  11. Select the kinds of private items you want to sync and click next. You do not have to make a selection here if you don't want your private records to sync up.
  12. Click Save and click Close
