Users with SAML SSO Can't Log In at login.salesforce.com or on Custom URL

Description
When users that are using SAML SSO cannot log in to Salesforce using login.salesforce.com or even custom URL, have the Salesforce admin check Setup > My Domain > Authentication Configuration. Then confirm if the Authentication Service Login Form checkbox is deselected. This permission hides the option for users to log in using username and password but does not entirely remove their ability to log in using Salesforce main page.
 
Resolution

To regain users' option to log in with their usernames and passwords, just select the Login Form checkbox then save the changes.

Otherwise, if the admin does not want to change any in My Domain settings, simply append '?login' at the end of login.salesforce.com. Apply the same on the custom URL if login.salesforce.com is set as prevented.
