The care team is the group of personal and professional caregivers who help a patient
complete their care plans. People who work for your organization are considered internal members
and have access to more information about the patient. External members, like family and outside
providers, have restricted access to the patient’s information based on the sharing rules set up
by your admin.

When you search for the person you want to add, check both the internal and the
external tabs in the middle of the page. The name you want can be on a tab that you’re not
currently viewing. If the person doesn’t appear in the results, then you can add a person by
creating an external member. If you get an error when you try to create an external user, ask
your Salesforce admin to configure Experience settings. They must enable the use of standard
external profiles for self-registration and user creation before you can create external users
here. Members without access to the Experience site can’t use Chatter to collaborate around
the patient’s care team.
Go to the All Care Plans subtab.
Expand your care plan and go to the Care Team tab.
Click the Actions button and select Add Care Team
Member.
In the popup window, you can either search for an existing user, or create a user to add to
your care team. Don’t forget to assign the care team role for this user.
Click Add.
Repeat steps three to five to add your entire care team.
Note Every care team must have a care coordinator.
