  3. From here, you can either add your problems, goals, and tasks separately, or apply a set of problems, goals, and tasks using a template. To apply problems, goals, and tasks separately, do this:
    1. To add problems, set the Group By view to Problem.
    2. Click the New Problem button.
    3. Enter a name, description, priority, and sort order for your problem and click Save.
    4. To add goals, set the Group By view to Goal.
    5. Click the New Goal button.
    6. Enter a name, description, priority, due date, progress percentage, and sort order and click Save.
    7. To add tasks, click the Add new task link in either the problem or goal view.
    8. Fill in the relevant details. If you want to associate your task with a specific problem or goal in your care plan, use the Problem and Goal lookup fields. But, don’t associate a task with both a problem and a goal.
    9. Click Save.
To apply problems, goals, and tasks using a template, do this:
    1. Click the Actions button and select Apply Care Plan Template.
    2. Select the templates you want and click Next. You can select up to 15 templates per care plan.
    3. Customize your selected templates, and click Next.
You can customize a Task Name and Due Date by clicking the Edit icon next to the corresponding field. To remove components and their child components that aren’t relevant to your care plan, deselect the checkbox next to the component.
    4. Review your problems, goals, and tasks, and click Apply.
