Care Performance Analytics Dashboards and Story
Care performance dashboards give care coordinators insight into patients’ adherence to the care plans. Care coordinators can use the insights to adjust future care coordination efforts. For example, care coordinators can identify patients who fall behind on the care plans and evaluate other data, such as patients’ risk profiles. Care coordinators can then focus on patients who’ve fallen behind and are identified as high-risk.
Care Coordinator Member Overview (Care Performance) Dashboard
Get insights into high-risk members and members who fall behind on their care plans, potentially exposing themselves to risk. Four visualizations give you details to manage risk in your patient population. Focus on the high-risk members with overdue care plan tasks. You can drill into the segments of your membership with overdue and open tasks, and tasks that are due soon. And dive into details by following links to the Members Care Plan Adherence Details and Members Goals and Tasks dashboards. Adjust the Risk Level filter to focus on high-, medium-, or low-risk population segments.
Members Care Plan Adherence Details Dashboard
Go to the Care Plan Adherence page to identify which patients are falling behind in their care plans by looking for members who have overdue tasks and goals. Care coordinators can focus on high-risk patients by checking how long care plan tasks have been overdue. Giving attention to these patients can help prevent avoidable readmissions. See details about each patient in the table and use the insight gained to help patients follow the plan. Coordinators can also create a list of high-risk patients of interest with Salesforce bulk actions and follow up with these patients to find out why they were unable to adhere to their plans, and even set up interventions.
Member Details shows details of the patients by their risk profile and insurance type.
Members Goals and Tasks Dashboard
Go to the How Many Goals are Due Soon? page to gain insights into patients who have care plan tasks and goals that are due soon. You can then contact those patients to set up appropriate interventions, thus ensuring that their care plans stay on track.
Go to the How Many Goals are Overdue? page to identify at-risk patients by identifying which of your patients have fallen behind on care plan tasks and goals. Then prioritize outreach to these patients and set up the appropriate interventions to help them avoid readmission.
Care Plan Adherence Story
This Einstein Discovery story gives care coordinators insights to improve patient engagement. The insights can enhance the processes coordinators already use to help patients manage their care plans. The story gives care coordinators insights into the variables that impact their patients’ care plan adherence, such as age or care plan type. Understanding these factors enables care coordinators to better engage with their patients, help them stay on track with care plan tasks, and improve their care outcomes.
Care (Embedded) Dashboard
Go to Account (patient or member) pages to access this dashboard after your admin embeds the page in the Account page layout. Use the page's metrics to measure a specific patient’s adherence to their care plan tasks and see the status of their care requests. Use these insights to engage appropriately with the patient and deliver personalized healthcare.
The dashboard shows these carousel cards with arrows that you can click to move from one card to the next:
-
- How Many Care Plans are Due Soon?
- Shows whether a patient is falling behind on care plans. Care coordinators can view the card to identify patients that are falling behind and take necessary steps to keep them on track.
-
- Care Request Type
- Shows a member’s open care requests and how long each has been open. Utilization managers can view this card and prioritize care requests for the member. For example, they can either focus on requests that have been open for a long time or on urgent requests.
Medication Management
The Medication Management dashboard contains metrics and visualizations to identify and analyze the medication adherence pattern, common health conditions, and medication distribution details of a group of patients. You can filter metrics and visualizations by status, patient age group, patient billing country, or start date. The dashboard shows the metrics, such as Count of Patients, Count of Patients with Active Medication, Count of Medicines in Use, and Count of Health Conditions for the filtered group of patients. The visualizations include the distribution of patients by gender, health conditions, plans and programs, and medication adherence details. The table shows specific patients' medication, adherence details, and health conditions.
Medication Analysis (Embedded)
The Medication Analysis Embedded dashboard shows the visualizations to quickly identify the common medication and the type of care plans followed by patients with similar health conditions. With this dashboard, providers and care coordinators can quickly compare a patient's medication details with similar patient data to achieve more effective health outcomes.
