Make sure that your admin completed the steps in Set Up Authorized Representative.
Log in to the Benefit Assistance Portal as a user who has Authorized Representative access.
-
On the portal home page, in the Find and Apply for Benefits card, select Apply for Someone Else.
The card also offers Apply for Myself, for constituents who apply on their own behalf.
-
In Applicant Information, complete the Common Questionnaire with the details of the person whom you represent.
Complete these required fields: First Name, Last Name, Gender, Date of Birth, Phone Number, and Preferred Communication Method.
-
In Evaluate Benefits Eligibility, complete the eligibility run, select the benefits to
apply for, and then select Proceed.
Selecting a benefit adds three sections to the application: the benefit's section, Authorized Representative, and Applicant Acknowledgement and Details Submission.
-
Open Authorized Representative, and under Authorized Representative Consent, select the consent statement that applies.
The statement names the authorization tier that it covers.
-
Under Authorized Representative Details, enter your First Name and Last Name.
Full Name appears automatically. You can also add Salutation, Middle Name, Date of Birth, Phone Number, and Email.
-
In eSignature, select the certification statement, add your signature in the Signature box, and then select Next.
To sign again, select Clear Signature.
-
Complete the Primary Applicant section.
That section presents one consent statement for each authorization, each with its own signature box and its own consent record.
- In Applicant Acknowledgement and Details Submission, open Acknowledgment, and complete Final Check.
- Select the confirmation statement, enter Name, optionally enter Place, add your signature, and then select Done.
- Select Submit.
You can submit an application even if you don't have an active authorization on file. If a prior authorization expired, use this flow to renew it as part of the application. After you submit, the application becomes read-only. You can't change any field, and your consent selection stays as you submitted it.
An authorization request goes to the agency for review with a Response Status of New. As the agency acts on the request, the status becomes Seen. It then becomes Signed if the agency grants the authorization, or Rejected if the agency doesn't.
