In Salesforce Classic, you create a care plan in three separate steps. First, you create
a case record for your care plan. Then you add a care team to this case record. And then
finally, you create problems, goals, and tasks for your care plan.

Add a Member to the Care Team The care team is the group of personal and professional caregivers who help a patient complete their care plans. People who work for your organization are considered internal members and have access to more information about the patient. External members, like family and outside providers, have restricted access to the patient’s information based on the sharing rules set up by your admin.
Add Problems, Goals, and Tasks Problems, Goals, and tasks create the basic structure for your patient or member’s care plan.
