            Preventive Care Agreement Fields

            Use these fields to define payment terms for a contract of agreement type Preventive Care Agreement. This agreement type supports the Preventive Care payment type in which providers are paid a bonus amount when they reach an agreed upon target. Targets can include achievements such as reducing admission or readmission rates to a certain percentage.

            Field Description
            Contract Payment Agreement The related contract payment agreement record.
            Bonus Measure Type The measure type used to define the bonus payout. For example, Preventive Readmission Rate or Preventive Admission Rate.
            Bonus Measure Description The description of the bonus measure type.
            Current Measure Percent The current percentage of the specified measure.
            Target Measure Percent The expected percentage of the specified measure.
            Bonus Payout The bonus paid on reaching the target measure percentage.
            Additional Information Additional information on the bonus payout and target expectation.
