To accurately track and report on action plans based on their status, set the Status
field on the Action Plan object page layout to Required.

From Setup, open Object Manager and click Action Plan.
Open Page Layouts and click Action Plan Layout.
Click the wrench icon next to Action Plan Status, select Required, and save your
changes.
