The Data Model
Use the Disease Surveillance data model to track disease data and public health activities. Capture patient-related information such as specimens, medication, demographics, disability status, and disease-related information such as disease definition, investigation, and outbreaks.

Define diseases using these objects:
| Object | purpose |

| Disease Definition | Represents a disease definition that public health organizations regularly monitor to identify and prevent outbreaks. |
| Disease Definition Criteria | Represents disease definition criteria, such as clinical, laboratory, and epidemiologic linkage used to diagnose diseases. |
| Disease Definition Condition | Represents a condition for disease definition criteria. |
Track disease surveillance activities and events using these objects:
| Object | Purpose |
|---|---|
| Contact Encounter | Represents a period of time during which two or more people were in close proximity to each other. |
| Contact Encounter Participant | Represents a person who was present during a contact encounter. |
| Disease Investigation | Represents an investigation of a disease. |
| Disease Investigation Case | Represents a junction between a disease investigation and a case. |
| Disease Outbreak | Represents a disease outbreak monitored by the public health organization. |
| Visit | Track information related to a field rep’s visit to any facility medical or non medical to conduct inspection. |
Record patient data using these objects:

| Clinical Service Request | Represents requests for a procedure or diagnostic service that needs to be planned, proposed, or performed as part of the patient’s healthcare. |
| Clinical Service Request Detail | Associates records from different objects to a clinical service request record to add more information to it. |
| Contact Profile | Represents information about an individual, such as their ethnicity, citizenship, birthplace, and race. |
| Health Condition | Represents a problem definition for a patient indicating the disease of a patient |
| Medication Administration | Represents an instance of medication administered to a patient. |
| Medication Administration Detail | Represents the details of an instance of medication administered to a patient like performer of medication administration |
| Problem Definition | Represents a problem definition. |
| Problem Definition Relationship | Represents the junction between parent problem definitions and related problem definitions. |
| Specimen | Represents a human or non-human sample used for laboratory testing or analysis |
