Care managers often spend significant time manually analyzing lengthy assessments, which can contain up to 400 questions. The Assessment Summary feature automates this analysis by transforming long lists of questions and answers into a concise, actionable summary. This feature helps care teams quickly identify patient needs, compare past and present health statuses, and take immediate action.

How Assessment Summarization Works

  • Generate Summary
    • The Summarize button is available on completed assessments and acknowledgment pages. It triggers the system to gather current and previously submitted assessment questions and responses.

    • The system generates a secure PDF document and sends the document as a source to the Generate Assessment Summary prompt template. This template pulls data from the Account ID, the generated assessment PDF, Member 360 data, and configured quick actions to generate the summary. You can update the prompt template as per your business needs. For example, you can update the instructions, add more sections, or generate only one key insight instead of three.

For more information on the LLM models supporting PDFs, see LLM Model Support.

  • View Summary

The assessment summary is displayed using a flexible, easy-to-read layout on the patient's record:

    • Key Drivers: Highlights critical patient details, including active conditions, symptoms, and medications.
    • Historical Comparisons: Compares the current assessment against past submissions so care teams can track progress.
    • Navigable Sources: Provides links to the original answers for quick verification.
    • Dynamic Layout: The summary displays as a series of cards. If the prompt template generates more than three cards, the layout automatically accommodates horizontal scrolling.

To ensure fast load times and save processing resources, the generated summary is saved directly to the Assessment Summary field on the Assessment object.

  • Take Action
    • Suggested actions are generated along with the summary as next steps for the care team. Your care managers can either accept or reject a suggestion.

    • The feature includes two built-in actions for care plans: Update Care Plan and Create Care Plan. These actions streamline your workflow by skipping redundant steps (like selecting a template) and taking you directly to the final confirmation page.
    • You can also create custom global actions and object-specific quick actions as required.
  • Enable Generative AI Assessment Summaries
Turn on the required org preference and select the prompt template used to generate assessment summaries. These settings enable the feature for users who have the required access. The selected prompt template determines the instructions and context used to generate an assessment summary.
  • Assign Permission Sets for Assessment Summaries
Assign permissions according to each user's responsibilities. Care coordinators need the appropriate permissions to generate a summary and manage suggested actions.
  • Customize the Assessment Summary Flexcard Component
Clone and configure the Generative AI Assessment Summary Flexcard to manage suggested actions and custom components. You can either show or hide the Suggested Actions section as per your business requirements. Similarly, you can replace the existing component to use your own custom component. Activate your tailored design to equip care managers with instant and more relevant AI-driven patient assessment insights.
  • Configure the Acknowledgment Page for Assessment Summaries
Verify and update all your existing assessment Omniscripts to include the custom Lightning component and show the Summarize button on the final acknowledgment page. Give care managers instant access to consolidated patient insights immediately after assessment completion.
  • Set up Suggested Actions for Assessment Summaries
Map your decision tables and MCG integration definitions within the Integrated Care Management settings page to trigger automated suggestions from assessment summaries. By default, all global actions and custom actions for creating and updating care plans are available as suggested actions. In addition, you can create custom global actions and object-specific quick actions to enable seamless updates directly from the summary.
