          Add Goals to an Existing Care Plan

          You can add more conditions to an existing care plan from the care plan interface on the patient account, case, or clinical service request record page.

          User Permissions Needed
          To add a goal Create permission on Goal Assignment and Goal Assignment Detail
          Read and View All Records permissions on Care Plan, Health Condition, and Goal Definition
          1. Navigate to the patient account, case, or clinical service request record page and go to the care plan interface.

            If you can’t find the care plan interface, talk to your Salesforce admin for help.

          2. In the Actions menu of the appropriate care plan, condition, or social determinant, click Add Goal.
          3. Enter a description.
            This text is displayed on the care plan interface as the goal’s name. So, keep the value in this field short.
          4. If the goal you’re adding already has a goal definition in the PGI library, select that problem definition.
          5. Set a priority and status.
          6. Select a start date and due date.
          7. Specify the progress of the goal.
          8. Specify the sort order of the goal.
          9. Select the person that the goal is assigned to.
          10. Click Add.

          Now that you’ve added a goal, consider adding a few interventions to the care plan.
