          Create a Care Plan from Care Gaps

          Create a care plan for a patient using open care gaps. If the care gaps are associated with problem definitions, you can also add goals and interventions to the care plan.

          Required Editions

          Available in: Lightning Experience

          Available in: Enterprise and Unlimited Editions with Health Cloud

          User Permissions Needed
          To create a care plan Create access on Care Plan and Case
          To use care gaps to create a care plan Read and View All Records access on Care Gap
          1. Navigate to the care plan user interface in the patient record page and click New Care Plan.
          2. Select Care Gaps as the care plan creation method.
          3. Click Next.
          4. Select one or more care gaps.
          5. Click Next.
          6. Enter the care plan name.
          7. Optionally, select the start date and end date.
          8. Specify the case number and the clinical service request name that you want to map to the care plan.
          9. If you selected a care gap that has a problem definition with hierarchy, expand that care gap.
          10. Select the goals and interventions you want to add to the care plan.
          11. Click Save.

          You see the newly created care plan at the top of the list in the care plan interface. When you expand the care plan, you see the associated care gaps.
