To evaluate a measure
Read access on Clinical Measure
To create a care gap
Write access on Care Gap
Navigate to the care gap interface in the patient record page and click New Care
Gap.
Select an active measure to evaluate.
Click Next.
Select a reporting period and review the measure details.
Click Evaluate Measure.
You can see the newly created care gap at the top of the list in the care gap interface.
If the evaluated measure has an associated problem definition, the care gap name is the same as
the associated problem definition name. If the measure has no associated problem definition, the
care gap name is the same as the measure name.
Note You can re-evaluate or manually close an open
care gap directly on the interface by clicking for the care gap you want to
edit.
