Create and Manage Outreach Campaign for Care Gap Closure
Use the Care Gap Closure Outreach Template to create a campaign that automatically sends
personalized communication to members with one or more open care gaps. When a member receives an
email, SMS, or both, they are reminded to complete their outstanding care tasks and close their
care gaps.
Required Editions
Available in: Lightning Experience
Available in: Enterprise and Unlimited Editions with Health Cloud,
Agentforce for Health Cloud, and Data Cloud add-on licenses
From the home page of the Health Engagement app, navigate to the Patient and Member
Outreach component, and click Start.
The outbound engagement creation window opens.
Select Care Gap Closure Outreach Template as the engagement template to base your campaign
on.
Click Next.
Enter the details of the campaign such as the campaign name, type, start date, and end
date.
Select Specific Segment as the audience type.
Search for and select Care Gap Closure Segment as the Data Cloud
segment.
Click Done.
The care gap closure campaign is created based on the Care Gap Closure Outreach Template.
The Health Engagement: Close Care Gaps flow sends a detailed email reminding the member to close
their open care tasks. This email contains the list of open care tasks and educational resources.
If the member doesn't respond after three days, the flow sends an SMS reminder. If the member
still doesn't respond, a follow-up task is automatically created for the care
coordinator.
