            Add Care Team Members to the Householding Map

            When a patient or member is created in Health Cloud, the householding map shows the person's name and the relationship groups associated with them: care plans, related accounts, and related contacts. When you add a care plan for the person, it appears in the map. Care plans show the members associated with each care plan that the person belongs to. You can add or remove care team members from a care plan.

            Required Editions

            Available in: Lightning Experience

            Available in: Enterprise and Unlimited Editions with Health Cloud
            User Permissions Needed
            To edit patient or member relationships: Health Cloud Standard
            1. From the care plan tile, click Add a Care Team Member.
            2. Enter the person’s name and role within the care team.
              If the person doesn’t exist in Health Cloud, add the person as a member.
            3. Click Add.

            To remove a member from the care plan, select Remove Member next to the member’s name.
