          Detect Clinical Issues with Patient’s Medications

          Proactively prevent medication-related adverse events and enhance patient safety by identifying any actual or potential clinical issues with your patient’s medications or treatment. When you detect clinical issues, such as incorrect dosage or treatment frequency, inform your patients and help them understand their medications and treatment better.

          Required Editions

          Available in: Lightning Experience

          Available in: Enterprise and Unlimited Editions with Health Cloud and the Medication Management Add-On license

          User Permissions Needed
          To detect clinical issues
          • Create, Read, and Update permissions on the following objects:
            • Account
            • Medication Therapy Review
            • Medication Therapy Statement Review
            • Medication Therapy Statement Review Issue
            • Clinical Detected Issue
            • Clinical Detected Issue Detail
            • Medication Statement
            • Medication Statement Detail
            • Medication Request
            • Medication Dispense
            • Patient Medication Dosage
          • Run Flows
          1. On a patient’s Person Account record, go to the Clinical Detected Issue subtab under the Comprehensive Medication Review or Targeted Medication Review tab.
          2. Click New Clinical Issue and add your observations.
            1. Select the identification start and end dates.
            2. Select the status, severity, and category of the issue.
          3. Add additional details and notes about the issue.
          4. If the issue is medication-related, select one or more medications from the Related Medications section.
          5. Save your changes.

          That’s it. The clinical issue you identified during the review is now recorded and added to the patient’s medication therapy review record. These details are also shared as part of the summary document that’s sent to the patient at the end of the comprehensive medication review.
