Manage Care Barriers
Add information to care plans about the obstacles that prevent participants from meeting their goals and using their benefits.

Even with the best intentions, events and circumstances can prevent individuals from using their assigned benefits and accomplishing their goals. For example, lack of transportation or childcare, unplanned work shifts, injury, and illness can all block people from attending counseling sessions, mentoring sessions, and job-training programs.
Define the types of care barriers that represent these and other categories of obstacles, and create care barriers to capture the details of specific obstacles or circumstances that affect a care plan participant. Then, relate a care barrier to an individual’s care plan by using a care plan detail record, or to their goal assignment by using a goal assignment detail record. As needed, adjust benefits or assign new or different benefits to the individual as needed to accommodate their barrier to care.
Create Care Barrier Types
Define categories for common obstacles or circumstances that make it difficult for individuals to achieve their goals or desired outcomes.
- From the App Launcher, find and select Care Barrier Types.
- Click New.

Enter a name for the barrier type.
For example, enter Transportation, Childcare, or Work Schedule.
- Select Active.
- If the care barrier type is related to a code set, select a code type and a code.
- Save your changes, or save them and create additional care barrier types.
Create Care Barriers for Care Plan Participants
Define a specific care barrier for a care plan participant.
- From the App Launcher, find and select Care Barriers.
- Click New.

Select a care barrier type.
For example, select Transportation.
- In Name, briefly describe the specific barrier for the individual. For example, enter Car Breakdown.
- In Patient, search for and select the account for the affected care plan participant.
- Select a status.
- Select Active.
- If needed, enter a description, and specify the duration of the barrier by selecting the Effective From and Effective To dates.
- If needed, search for and select the case related to the individual’s care plan.
- Save your changes.
Add Care Barriers to Care Plans
To track someone’s care barriers in a care plan, add the barrier to their care plan details.
- From the App Launcher, find and select Care Plan Details.
- Click New.
- Search for and select the care plan that you want to relate to the care barrier.
- In Detail Record, select Care Barrier, and then search for and select a record to look up to.
- If the care barrier type is related to a code set, in Detail Code, select a code set or code set bundle, and then search for and select the code record.
- Select a detail type and problem priority.
- If you're tracking multiple care barriers on the care plan and they have dependencies, enter a sequence number.
- Save your changes.
Add Care Barriers to Goal Assignments
To track someone’s care barriers as they relate to their goals, add the barrier to their goal assignment details.
- From the App Launcher, find and select Goal Assignment Details.
- Click New.
- Search for and select the goal assignment that you want to relate to the care barrier.
- In Detail Record, search for and select a care barrier record.
- If the care barrier type is related to a code set, in Detail Code, select a code set or code set bundle, and then search for and select the code record.
- Save your changes.
