            Care Plan Creation

            Create goal definitions to define the objectives for individuals to reach by using a care plan. Create care plan templates for common assistance scenarios to give staff members a head start on creating individualized care plans. For example, create a care plan for individuals who need employment assistance. Add tasks to care plans by using action plan templates, and create a care plan for a specific case.

Use care plan templates to deliver benefits consistently and expediently. Talk to your Salesforce admin about being able to clone care plan templates.
            • Create Care Plan Tasks
              Keep staff members and the individuals that you support accountable and on track by adding tasks to the benefits and goals in a care plan.
            • Create a Care Plan for a Case
              Create personalized care plans based on care plan templates to help individuals that you support reach their specific goals.
