Create a Care Plan for a Case
Create personalized care plans based on care plan templates to help individuals that you support reach their specific goals.

When you create a care plan from a template for a specific case, it includes the goals, benefits, and tasks in the template.
Note In the care plan creation guided flow, the fields always appear in a fixed order. As a
result, sometimes they don’t reflect the customizations that you make to the Care Plan page
layout in Setup.
- From the App Launcher, find and select Cases.
- From a list view, select a case record.
- On the Care Plans related list, click Add.

Specify these details.

Enter a name.
For example, string together the surname of the primary case participant and the care plan template name: Marshall Healthy Household Care Plan.
- Select the primary individual related to the case as the participant.

To create a care plan from a template, search for and select a care plan
template.
Note You can select only published care plan templates. - Select Proposed as the status.
- If needed, specify the start and end dates, and enter a description.

Enter a name.
- Click Next.
- To customize the care plan for the case, add or remove benefits and goals, and then click Next.
- For each benefit and goal, in Enrollee, select or add a case participant.
- If needed, give your goals a custom name and add goals.

Add details to your benefits and goals.
For example, add a start date and a target completion date.
- Save your work.
To see a summary of the goals and benefits in the care plan, go to the Preview tab. To modify a care plan task or goal, click Quick Update.
