Care Plans Prerequisites
To enable and tailor care plans in Salesforce, turn on the feature and set up custom page layouts, including specific fields for unique needs. Also, create personalized goal names, define picklist values for care barriers, and add a benefit disbursements component for full tracking.

Enable Care Plans
Turn on the org setting that enables you to create and manage care plans.
- From Setup, in the Quick Find box, enter Program and Case Management, and then select Care Plan Settings.
- Turn on Create and manage care plans.
Set Up Useful Related Lists
Make it easy for intake officers, caseworkers, and other staff members to access the items that they need to work with care plans by adding useful related lists to pages. For example:
- On the Case page layout, add the Care Plans, Case Participant, and Case Programs related lists. The Case Programs related list includes an Enroll Participants button.
- On the Care Plan Template page layout, add the Action Plan Template Assignment, Care Plans, Care Plan Template Goal, and Care Plan Template Benefit related lists.
- On the Care Plan page layout, add Care Plan Details related list.
- On the Account page layout, add the Care Plans related list.
Set Up Custom Goal Assignment Names
Create personalized, meaningful, and engaging care plans that fit each client's unique needs with custom goals. By creating and entering a custom name for a goal, case managers can track goals specific to a client’s needs.
- Add the Custom Goal Name field to the Goal Assignment page layout.
- Add the Custom Goal Name Required field to the Goal Definition page layout. When Custom Goal Name Required is selected, the Custom Goal Name field is required for Goal Assignments associated with this Goal Definition.
Add Custom Fields for the Care Plan Flow
Build care plans tailored to your needs by adding custom fields to the guided flow that creates them. Create the fields on the Benefit Assignment, Care Plan, or Goal Assignment object.
For example, create a Language field on care plans to capture the language that a client or individual speaks. See Create Custom Fields.
- The Participant field always shows last in the guided flow.
- Start Date always precedes End Date in the guided flow.
- If the page layout includes fields in only one column, the guided flow shows the fields across two columns for optimal use of space.
- Sections in page layouts aren’t included.
Create Picklist Values for Care Barrier Objects
Care barriers have a Priority and Status. Care plan details have a Detail Type and a Problem Priority. Salesforce doesn’t provide default values for these fields.
Create the picklist values before you start using care barriers. For example:
- For care barrier Priority and care plan detail Problem Priority, create the picklist values High, Medium, and Low.
- For care barrier Status, create the values Proposed, Draft, Active, Completed, and Canceled.
- For care plan detail Detail Type, create the values Based On, Category, Contributor, and Supporting Information.

Add the Benefit Disbursements Lightning Component to Care Plans
The Benefit Disbursements component shows the status of the benefits assigned to a case participant, including the start and end dates for planned and disbursed benefit sessions, or payment amounts for monetary benefits. Make the Benefit Disbursements component available to users on relevant Lightning record pages such as Care Plan and Benefit Assignment.
- From the App Launcher, find and select Care Plans.
- Select a care plan record.

, and then select Edit Page.
- Select the Tabs component on the page canvas and, in the Tabs properties panel, click Add Tab.
- Click the new tab’s label, and select Custom.
- Enter Benefit Disbursements as the custom label, and then click Done.
- Select Benefit Disbursements on the page canvas, and then drag the Manage Benefit Disbursement component to it.
- Save your changes and activate the page.
- Similarly, add the component to the benefit assignment Lightning record page, if needed.
