          Case Referral Intake Guided Flows Setup

          Help individuals who seek referrals for support services and social and educational programs by configuring the flows that streamline referral requests.

          Configure these Omniscript guided flows so that intake officers can easily process referral requests from clients.

          Submit a Referral
          Collect information about a referral request. Use predefined questions to capture basic details about the client and their situation to share with the provider. And use custom assessment questions to collect additional relevant information depending on the type of referral requested.
          Edit a Referral
          Add or change the details of a referral, or add notes and supporting documentation.

          Note You can’t edit submitted assessments.
          Create a Referral Case
          After a referral request is screened, create a case from a referral.
          • Case Referral Intake Guided Flows Components
            Review the Omnistudio components that the Case Referral Intake guided flows use.
          • Set Up the Case Referral Intake Flow
            Create assessment questions, grouped into assessment question categories, that capture information about clients who seek a referral to a service, provider, or program. Add the assessment questions to Omniscript forms—one form for each category—and then add the forms to the referral intake Omniscript guided flow. Create a button that intake officers use to start the referral request guided flow. And configure a Flexcard that makes it easy for intake officers or case managers to create accounts and contacts for case participants.
          • Set Up the Edit Referral Flow
            Activate the Omniscript for editing a referral by using a guided flow, and configure the button that users click to start the flow.
          • Set Up the Create Case from Referral Flow
            Activate the Omniscript flow that creates a case from a referral, and configure the button that users click to start the flow.
