            Create a Survey for Patient Feedback

            Use Salesforce Feedback Management to create patient and member surveys.

            Required Editions

            Available in: Lightning Experience

            Available in: Enterprise and Unlimited Editions with Health Cloud
            User Permissions Needed
            To create a survey:
            • Manage Surveys
            • Read, Create, Edit, and Delete permissions on Surveys and Survey Invitations objects
            1. On the Surveys tab, click New to open the Survey Builder.
            2. Enter a name for the survey.
            3. Edit the survey’s title page.
              Note
              Note The first slide of every survey is a title page, not a question page.
            4. To add a question page, click Add Page in the sidebar.
            5. To add one or more questions to the page, click Add Question.
            6. Customize the survey’s thank you page.
            7. Do one of the following.
              1. Optional: To continue working on the survey later, click Save.
              2. Optional: To review how the survey looks, click Preview.
              3. To activate the survey so that you can send it to participants, click Activate.
