          Gather Feedback About Patients or Members

          Improve the quality of care by gathering feedback that helps to manage your patients or members more efficiently.

          • Gather Information with Surveys
            Surveys are a valuable way of efficiently gathering feedback from the people whose care you manage. From the console, it’s easy to send surveys to patients or members and have a record of the responses at your fingertips.
          • Create a Survey for Patient Feedback
            Use Salesforce Feedback Management to create patient and member surveys.
          • Complete an Assessment
            Quickly locate the correct assessment and complete it by using the Assessment component. Specific record pages show assessments related to a particular individual. The home pages of apps such as the Behavioral Health app, Integrated Care Management app, and Crisis Support Center Management app show assessments for all the accounts assigned to you. If you must pause an assessment in progress, save it as a draft. You can easily find it later and complete it at your convenience.
          • Track Assessment History
            To gain a better understanding of a person’s performance and progress, track assessment history. You can see details such as submitted date, user, and score trends using the Assessment History component.
          • Track Assessment Questions Score Changes
            Get detailed insights into a person’s responses by using the Assessment Questions component to track changes in each assessment question score.
