Create a Form in the Form Builder in Agentforce Operations
Use the form builder to create a reusable formatted collection of fields for a task in Agentforce Operations.

A form is a collection of fields with optional formatting that’s attached to a task. Use a form to reuse the same fields in multiple tasks or to control the layout and presentation of the information you're requesting from task assignees. To create a form in the form builder:
- Create a blueprint or edit an existing blueprint.
- In the blueprint, create a task by clicking + Task or click the existing task where you want to add the form.
- In the Request information section, click + Add form.

Scroll to the end of the list of forms and click +Create new form.
The form builder opens.
- To add formatting and layout, click the options in the Layout section.
- To add fields to the form, search for and click fields.
- If you don’t find the field you want, you can create a new field by clicking + Create. Learn more in Create Custom Fields in Agentforce Operations.
- To add multiple sections, click + Add section.
- When you complete your form, click Close & continue.
Agentforce Operations attaches the form to the task and shows it to assignees when they complete the task. You can now select the form from the list of forms in another task.
