            Deploy this template to provide employees with a standardized way to request for Multi-Factor Authentication (MFA) reset in case of lost or compromised credentials.

            This template creates a service request record that captures essential user details for accurate and auditable fulfillment. Review what’s included with the template.

            Intake Attributes

            The intake form for this template captures these details from the employee:

            • Reason for Reset: A brief explanation for the MFA reset request, such as a lost or damaged device, a new device setup, or issues with the authentication app.

            Automated Fulfillment

            This service process includes a fulfillment flow that automatically processes the service request. You can extend this flow in Flow Builder to include custom logic, such as automated manager approvals or inventory checks.

            Integration

            This template uses a preconfigured integration with Okta in the fulfillment flow. To use this integration, ensure your Okta credentials are configured. To learn more about this third-party connector, see Okta Connector.
