This template creates a service request record that captures essential user details for accurate and auditable fulfillment. Review what’s included with the template.
Intake Attributes
The intake form for this template captures these details from the employee:
- Location: The Azure region where the virtual machine will be provisioned.
- VM Size: The specific virtual machine size required.
- VM Image Publisher: The publisher of the software image used to create the virtual machine.
- VM Image Offer: The specific image offer for the virtual machine.
- VM Image SKU: The specific SKU version of the selected image.
- VM Name: The unique name assigned to the new virtual machine.
- Virtual Network Name: The name of the virtual network to which the virtual machine will be connected.
- Subnet Name: The specific subnet within the virtual network for the virtual machine.
- Network Interface Name: The name of the network interface used by the virtual machine.
- Admin Username: The username for the admin account on the virtual machine.
- Admin Password: The secure password for the admin account.
- Business Justification: A brief explanation of why the new virtual machine is required.
Automated Fulfillment
This service process includes a fulfillment flow that automatically processes the service request. You can extend this flow in Flow Builder to include custom logic, such as automated manager approvals or inventory checks.
Integration
This template uses a preconfigured integration with Microsoft Azure in the fulfillment flow. To use this integration, ensure your Microsoft Azure credentials are configured. To learn more about this third-party connector, see Microsoft Azure Connector.
