Configure Data Mappings for the Wellness Questionnaire

The Wellness Questionnaire comes preconfigured with default data mappings that map Wellness Questionnaire survey responses to fields on the Health Verification record. When a respondent submits the Wellness Questionnaire, Safety Cloud generates a Health Verification record and, using the questionnaire’s data mappings, maps the survey responses to the matching fields on the record.

To view and configure the Wellness Questionnaire default data mappings, complete these steps.

  1. To ensure that you’re using an inactive version of the Wellness Questionnaire, from the dropdown menu, select Create New Version.
  2. From the dropdown, select Advanced Settings, and select Survey Data Mapping.
    The Survey Data Mapping page is displayed. The page shows the default data mapping object named Questionnaire_To_HealthVerification that comes with the Wellness Questionnaire.
  3. Edit the default data mappings in Questionnaire_To_HealthVerification by clicking the pencil icon.
    The Edit Mapping page is displayed.
  4. Configure the mappings between survey responses on the questionnaire and fields on the Health Verification record, and save your changes.
  5. To create more mappings between a survey response and a field on the Health Verification record, click Add Row.
    1. Specify the field, information type, and the value from the survey.
    2. Save your changes.
    If you want to map a survey response to a particular Occasion, the mapping row contains these values.
    • Field: Occasion Id
    • Information Type: String
    • Value: Occasion Id, such as a0B7x0000007KGXEA2

    For this example to work, you must first create an Occasion record in Safety Cloud to get the Occasion ID value.

    Note
    Note To keep the Wellness Questionnaire configurable and extensible for any implementation, we don’t include Occasion ID in the default mapping for the Wellness Questionnaire. Additionally, Occasion Id isn’t a required field on the Health Verification record.

  6. Optional: Stamp the Wellness Questionnaire with the OccasionId dynamically.
    Stamping the Wellness Questionnaire with the OccasionId allows you to locate and map Health Verification records with their associated OccasionId.
    1. From your version of the Wellness Questionnaire, go to Survey Builder | Merge Field | Org and Custom Variable, and set these fields.
    2. Click New Variable, then add the Variable = varOccasionId, and Type = Custom.
    3. Save your changes, and click Back to Builder.
    4. Go toAdvanced Settings Survey Data Mapping, and click the pencil icon.
    5. From Define Target Object Fields and Values, add a row.
      • Field: Occasion Id
      • Information Type: Custom Variable
      • Value: varOccasionId
    6. Ensure that the URL parameter includes the Occasion Id value in OccId.
    The OccasionId is stamped to the Health Verification record.

    Any system that reads a QR code must read these four pieces of information.

    • Verification ID.
    • Verification Start Date and Time.
    • Verification End Date and Time
    • Occasion ID

    Dates are for offline support. If your event has unstable WiFi, the QR codes can be scanned using the start and end dates or times.

    You can configure these fields so that the QR code is only valid for a specific set of hours, as shown in the sample.

    • Access Start Time = Beginning of day for negative test (the day of the event)
    • Access End Time = End of day for day of negative test (the day of the event)
    • Access Offset= Time in minutes after a user’s health status is verified
    Note
    Note You can use the Access Offset field or the Access Start Datetime and Access End Datetime fields to set the time range for which the generated QR code remains valid. You can’t use both.
