Customize the Wellness Questionnaire

You can customize the Wellness Questionnaire by including the name of the event and the first and last name of the individual who is filling it out.

Required Editions

Available in: Enterprise and Unlimited Editions
User Permissions Needed
To install a package: Modify All Data

To customize the Wellness Questionnaire, complete these steps.

  1. Activate the version on the top right of the page.
  2. Generate the URL for the questionnaire by clicking Send.
  3. Customize the questionnaire by parameterizing the URL.

    You can customize the survey by passing URL parameters for variables, such as the Event Name and Attendee First and Last Name.

    Parameterized URL Example:
