You can access and set up a Wellness Questionnaire for Health Verifications.

For information on how to edit Surveys, refer to Create and Design Surveys.

To access and set up the Safety Cloud Wellness Questionnaire, complete these steps.

From the App Launcher, enter Surveys, and then select Surveys.
Find the Wellness Questionnaire, and select it.
Click Open Latest Version.
To view a page in the Wellness Questionnaire, from the navigation on the left, select the page.
To create an editable version of the questionnaire, from the dropdown menu in the upper right corner of the user interface, select Create New Version.
