Generate Evaluation Form

Use OmniStudio to package your active questions into a functional evaluation script.

User Permissions Needed
To build quality assessment scripts:	Quality Manager

From the Service Console, go to Assessment Questions and click Select Questions.
In the assessment, enter Quality Management and add provide these details:
- Name: A unique name
- Language: English
- Type: Enter a type such as Quality
- Sub Type: Enter type such as Form
For a given combination of Name, Type and Sub Type, only one evaluation form version must be active.
Select an assessment question category in the Create Form tab and drag the questions into the OmniScript tab.
Click Add Steps to structure the steps into logical sections to guide the analyst and use names for categories and steps.
To build the evaluation form, select Build OmniScript.
Preview the evaluation form.
Submit the changes and Activate the form.

You can create and manage any number of evaluation forms for different assessment needs.
