Provider Card Fields
Use the fields provided on the card setup page to configure your provider cards the way you want them.
| Field | Description |

| Activate card? | Make this card available for inclusion on Account or Contact record pages. |
| Name the Card | Give the card a name that describes the two sides of the relationship it represents. |
| Reference Name | Name that appears in Setup. |
| Developer Name | Name used for API-level interaction. |
| What page will this card appear on? | Cards can appear on an Account page or a Contact page. You’ll usually want a card for a practitioner to appear on the Account page of the facility where they work, and a card for a facility to appear on the Account page of a practitioner who works at that facility. For staff or physicians modeled as contacts, it can also appear on their Contact record page. |
| Select Object | Select the object whose record pages the card will appear on. |
| Select Record Type | Select the record type that this card will display on, if any. |
| How is this card related to the page where it appears? | Same object: All the information you want to display is in the object where the card is shown. For example, you are displaying the card on a facility’s Account page and it contains the facility’s address and phone number, which are stored in the facility’s Account record. |
| Directly: The information you want to display is in an object that's directly related to the current object. For example, your facility card displays the name and contact information for the person who books appointments at the facility. The Account object has a lookup field to the Contact object, which contains that information. | |
| Indirectly:The information you want to show is in a junction object that connects the base object to the page where the card is displayed. For example, Healthcare Practitioner Facility is an object that matches practitioners with the places where they work. | |
| Which object holds the principal information this card will display? (This object is the hub from which you will jump up to three hops away in object relationships.) | For example, if the main point of the card is to show the operating hours and contact information for the facility, choose Healthcare Facility. If the main point is to show a provider’s qualifications, choose Accreditation. |
| Select Object | Select the object where the main information is stored. |
| Select Record Type | Select the record type that this card will display on, if any. |
| Source Object | The relationship field from the object named in "Select Object" to the respective Account or Contact. |
