Medication Therapy Management

Medication Therapy Management offers a range of services to help patients optimize their medication regimen for the best possible therapeutic outcomes, reduced medication-related adverse events, and minimized healthcare costs.

The Medication Therapy Management program includes two review types:

• Comprehensive Medication Review

  A yearly, all-inclusive review of an enrolled patient. The structured and extensive evaluation process thoroughly analyzes all of a patient’s active medications and relevant medical conditions. Patients are assigned to-do lists with action items to help improve their health outcomes.

• Targeted Medication Review

  A focused review that identifies actual or potential problems with specific medications in the patient’s medication regimen. The review’s ongoing monitoring approach is a cost-effective option that helps to identify and proactively prevent or resolve medication issues. Targeted Medication Review can be conducted quarterly or whenever required, such as when a patient is discharged from a hospital, or if a health issue is identified.

Patients are enrolled in a Medication Therapy Management program based on eligibility criteria such as count and cost of their medications, chronic diseases, and more. Enrolled patients can opt out of the program at any time.

Medication Therapy Management works seamlessly with other Health Cloud features to conduct effective reviews.

• Use the Care Programs feature to create a program for enrolling patients that’s specific to medication therapy review.
• When combined with the Patient Medication Manager lightning component, the built-in Start Medication Therapy Review flow makes the review initiation process seamless. Everything from identifying the patient’s cognitive status and health conditions causing the impairment to gathering information from the surrogate decision maker, is easy to record.
• Medication therapy management integrates with the Care Plan, Care Plan Activity, Care Plan Template, and Action Plan Template Assignment objects for smooth management of patient’s to-do lists. Simply configure a set of tasks for each healthcare issue and your users can directly assign these predefined tasks to patients.
• Use Data Processing Engine or Business Rules Engine to define the eligibility rules for the medication therapy management program.
