To support tracking score history, enable history tracking for the Score and Date field
in Health Score. The Score Details for Unified Health Scoring component can’t plot the score
change over time graph without tracking score history.

User Permissions Needed
To enable history tracking:
Customize Application
From the Object Manager, enter Health Score in the Quick Find box,
and then select Health Score.
Click Fields & Relationships.
Click Set History Tracking.
Select Score and Date and save your changes.
