Evaluate Clients and Identify Risks by Using Screening Assessments

Gain a better understanding of a client’s risk using screening assessments. Assessments are useful tools that help you determine the behavioral state of a client. They help you decide which treatments and services are most suitable for them. They also help identify warning signs and the strategies clients can adopt to cope with a crisis.

          Required Editions

          Available in: Lightning Experience

Available in: Enterprise and Unlimited Editions with Health Cloud or Life Sciences Cloud

You can launch assessments from the Action Launcher component in a case record page. If your Salesforce admin has pinned frequently used assessments in the component search bar, you can launch them with just one click. Specific record pages show assessments related to a particular individual. The home page of the Crisis Support Center Management app shows assessments for all the accounts assigned to you.

          Intake Assessment

The Intake Assessment is usually the first assessment you’d administer to a client. It helps you evaluate the client’s mental and behavioral state, so that you can decide how to proceed with their case. It asks questions such as a client’s gender identity, what their main concern is, and if they’re having suicidal thoughts. A client in crisis may not always be able to express themselves clearly and coherently, and this assessment can act as a guide in such cases.

          Immediate Risk Assessment

The Immediate Risk Assessment helps you identify high risk clients. It determines whether a client has had suicidal thoughts, how often they have such thoughts, and if they intend to act on them.

          Safety Plan Assessment

The Safety Plan Assessment helps a client identify their warning signs, coping strategies, distractions, supports, professional supports, and safer environments. On completion of the assessment, a safety plan is generated in a shareable PDF format. You can share this file with low-risk clients who aren’t an immediate danger to themselves or others. High-risk clients who require further treatment and services may not benefit as much from this assessment.

You can find ongoing and completed assessments for each case under Assessments in the Related subtab of the case record page.
