providers are paid based on the percentile range for that member. Each percentile range has a fee
that is determined by factors such as the member's age.

The price for the procedure if the member falls into the second percentile bracket
based on the payer's determining factor.
Third Bracket Price
The price for the procedure if the member falls into the third percentile bracket
based on the payer's determining factor.
Fourth Bracket Price
The price for the procedure if the member falls into the fourth percentile bracket
based on the payer's determining factor.
Fifth Bracket Price
The price for the procedure if the member falls into the fifth percentile bracket
based on the payer's determining factor.
Bracket Information
The description of the percentile bracket information.
Standard Price
The standard price for the procedure defined by the governing body.
Relative Value Unit
The relative value unit for the procedure.
