providers are paid a negotiated amount for each service they perform. For example, patient
visits. Fees are based on the comprehensive fee schedule definition set up by CMS (Centers for
Medicare and Medicaid Services).

Procedure Code
The industry standard code for the procedure, such as CPT or HCPCS.
Procedure Code Description

Non-Facility Price
The fees when the procedure is performed in a non-facility location, such as the
physician's office. This amount is determined by the fee schedule definition that’s
associated with this payment agreement record.
Facility Price
The fees when the procedure is performed in a facility location, such as a hospital
or Ambulatory Surgical Center (ASC). This amount is determined by the fee schedule definition
that’s associated with this payment agreement record.
