Enable seamless communication and care coordination between patients and their home
health agency by setting up a dedicated Home Health Experience cloud site for patients. Assign
the required permissions to patients, define sharing settings for the org, and configure the
Home Health flows. With these configurations, your patients can easily review and manage their
home visits, leading to an efficient healthcare experience.

Prerequisites:
Verify that Digital Experiences is enabled.
Select Allow using standard external profiles for self-registration, user
creation, and login under Digital Experience Settings.
From the Company Information page in Setup, ensure you have these org-wide user licenses:
Make sure you select the Experience cloud user profile that you
created. Also, assign the Home Health Patient permission set to the user
account.
After you create the user account, patients receive an email with the login
details and a link to reset the password. Patients can then use the new password to log in
to the Home Health patient portal.
Build an Experience Site for Home Health Patients Set up an Experience Cloud site with the Home Health Patient Visits lightning component to keep your patients connected with their home health agency. Patients can easily access the site from their personal devices to manage their past, current, and upcoming home visits.
Define Sharing Settings for Home Health Patients Give your patients access to the profile picture of their caregivers by defining sharing settings for the User object. When your patients access their Experience cloud site, they can view the picture along with the other home visit details such as visit date, time, and status.
Create a Custom Notification for Patient Requests Create a custom notification for sending in-app notifications to patients about their home visit requests. When a scheduler approves or rejects a patient’s request, the Notify Patients About Home Visits flow uses this custom notification to send a notification to the patient.
Clone and Activate the Notify Patients About Home Visits Flow Enhance patient engagement and improve care coordination with patient self-service capabilities by setting up the Notify Patients About Home Visits flow. The flow sends in-app notifications to patients and keeps them up to date about the approval or rejection of their home visit requests. Simply clone the latest version of the flow and activate it.
Activate the Schedule Home Healthcare Visit Flow Optimize the patient self-scheduling process and help your schedulers to easily schedule visits based on patient requests by setting up the Schedule Home Healthcare Visit flow. Simply activate the latest version of the flow and you’re done.
Activate the Reschedule Home Healthcare Visit Flow Streamline and expedite the home visit rescheduling process for reschedule requests raised by patients by setting up the Reschedule Home Healthcare Visit flow. Simply activate the latest version of the flow and you’re done.
