a patient receives. To ensure data reliability, Health Cloud restricts updates to certain fields

Whether a care gap is calculated, imported, or manually created, you can’t update the Record
Origin Type field after the record is created. The record origin type determines the actions you
can perform on other fields.
Consult this table to understand which actions you can perform on the Measure Evaluation Status
and Status Reason fields for each record origin type.
