Create a clinical measure record to define a clinical measure so that care managers can
evaluate it against a patient’s data in Health Cloud using an Omniscript. The Omniscript
automatically creates a care gap based on the evaluation.

To create a clinical measure:
Read, Create, and Edit access on Clinical Measures
For example, create a clinical measure record to define a hypertension HEDIS measure to assess
care gaps among patients whose blood pressure levels are outside the recommended threshold.
From the App Launcher, find and select Clinical Measures.
Click New.
Enter a name and version number for the clinical measure.
Select the status.
Select the effective start date and end date.
In Publisher Name, enter the name of the organization that publishes the clinical measure.
For example, enter HEDIS if this is a HEDIS (Healthcare Effectiveness Data and Information Set)
measure.
Optionally, fill in other fields such as the Description and Problem Definition
fields.
Click Save.
