Integrated Care Management Data Model and Permissions
Integrated Care Management uses an FHIR R4 and USCDI-aligned data model to store its data and make it interoperable.

Permissions
Users need the Care Plans Access permission set license to use Integrated Care Management.
Care Plan
Here’s the set of objects used in instantiated care plans and the level of access your users need for them.
| Object | Purpose | Required Access |

| Care Barrier | A social determinant of health that’s recorded in a care plan. |
|
| Care Barrier Type | Represents the different possible types of social determinants of health. |
|
| Care Plan | The high-level record that represents a care plan. |
|
| Care Plan Detail | The junction that associates a care plan to its related records. Its primary use is to connect a care plan to the Health Condition (problem) that it addresses. |
|
| Case | The case record that a care plan is created against. |
|
| Health Condition | A condition that a care plan is created to address. |
|
| Health Condition Detail | Additional details related to a condition, such as the condition's site and laterality on the patient’s body. |
|
| Goal Assignment | A goal that a care plan is intended to achieve. |
|
| Goal Assignment Detail | The junction that connects a goal to its parent problem. |
|
| Action Plan | A group of related intervention tasks in a care plan. An action plan is attached to a problem, goal, or care plan. |
|
| Action Plan Item | A child record of an action plan representing each of its interventions. These records reference Task. |
|
| Task | An intervention task in a care plan that’s meant to achieve a goal, resolve a problem, or act on a care plan. |

Care Plan Templates
Here’s the set of objects you use to define the hierarchical structure of care plan templates and the level of access your users need for them.

| Care Plan Template | The template for the high-level care plan. |
|
| Care Plan Template Problem | The template for problems attached to a care plan. These problems can be defined with or without goals. If the problem has no child goal, this template references a problem definition in the PGI library. If the problem has a child goal, this template references a problem goal definition junction. |

| Care Plan Template Goal | The template for goals attached directly to a care plan. This template references a goal definition in the PGI library. |
|
| Problem Goal Definition | The junction that associates a parent problem to its child goal. |
|
| Action Plan Template Assignment | The junction that associates an action plan template to its associated goal, problem, or care plan. |

Problem Goal Intervention Library
Here’s the set of objects that you use to define a PGI library for your users and the level of access users need for them.

| Action Plan Template | The definition record for a set of intervention tasks that’s instantiated by Integrated Care Management as an action plan record with its associated action plan item and task records. |

| Action Plan Template Item | A child record of an action plan template representing each of its interventions. These records are instantiated by Integrated Care Management as action plan item records. |

Care Gaps
Here’s the set of objects that you use to define a care gap and the level of access your users need for them.

| Care Gap | The record for gaps in patient care through a specified period. |
|
| Clinical Measure | The record for a structured and computable definition of a health-related measure used for identifying care gaps. |
|
| Clinical Measure Criteria | The record for a computable criteria of a health-related measure. |
|
| Clinical Measure Criteria Group | The record for a group of computable criteria for a health-related measure. |
|
| Care Gap Criteria Result | The record for the result of an evaluated criteria. |
|
In addition to these objects, Integrated Care Management also uses the objects in the Discovery Framework data model to support assessments and recommendations.
