Home Health Users
Users with different roles work on different aspects of Home Health. Based on your organization’s roles and personas, set up profiles for these users.
- Schedulers work desk jobs and schedule visits for home healthcare services.
- Care coordinators work desk jobs and manage quoting and budgeting of home healthcare services.
- Care resources work in the field and visit patient homes to provide care for patients. Depending on their level of access to clinical data, you can further classify care resources as clinicians and caregivers.
- Patients receive care at their home from clinicians and caregivers. Patients can use Home Health’s dedicated Experience Cloud site to easily manage their home visits and stay connected with their home health agency.
Field Operations
Care resources can use Home Health in the Field Service mobile app to keep track of their work in the field. With the app, care resources can see the visits that are assigned to them and the tasks they must perform during each visit. Care resources can also generate reports based on templates, and if they have the right permissions, they can see the patient’s clinical information.
Visit Types
Home Health supports two types of visits:
- Start-of-care visit for an initial assessment
- Recurring visits for providing the required services
With Home Health, your users can schedule the full series of recurring visits in one go. Home Health creates each individual visit and assigns the appropriate care resources while your users move on to the next task.
Visit Assignment
Home Health uses enhanced schedule optimization to ensure that care resources spend more time assisting patients and less time traveling to appointments. Home Health also automatically updates visit assignments when a care resource requests time off that conflicts with their visit assignments. To give schedulers enhanced visibility and greater control over care resource assignment, Home Health enables them to manually select the most suitable resource from a set of recommendations. Also, schedulers can reassign visits to different care resources in bulk.
Patient Eligibility Verification
Home Health helps users to expedite the scheduling of recurring visits with a simplified, time-saving solution for patient eligibility verification. When schedulers create recurring visits, Home Health’s built-in flow verifies whether the patient is eligible by checking for an active member plan and benefit coverage of type Home Health. If the patient is eligible, a message indicating the same appears. This automated background check helps schedulers to cut down on repetitive eligibility verification steps and helps them avoid any unintended manual errors during verification.
Electronic Visit Verification
Home Health’s comprehensive solution for Electronic Visit Verification (EVV) allows a home health agency to track care resource visits and verify that a patient receives the services authorized in their plans of care. This verification gives accurate and up-to-date visit data that subsequently reduces inappropriate billing, safeguards against fraud, and helps with future audits.
