Care barriers help you gain a comprehensive understanding of the challenges that clients face in receiving the crisis services they need. For example, you’re on a call with a client who shares that they haven’t checked in to the facility where you’ve referred them to treat their substance abuse. As you dig deeper, you find out that this client doesn’t have a car and public transport isn’t easily accessible from where they live. So, you record this challenge as a care barrier. Then, to address this barrier, you create an intervention task to sign your client up for a car service.

You can see the newly created social determinants of health records in the Care Barriers tab on the case record page and the page of the account record linked with the case.
